La terapia antifungina empirica viene largamente impiegata da due decenni nei pazienti affetti da patologie oncoematologiche in corso di febbre e neutropenia.1 L’elevata letalità correlata alle micosi invasive e l’utilità di un trattamento precoce hanno indotto ad adottare anche per gli antifungini la stessa politica già impiegata con successo nella terapia antibatterica. In realtà i risultati positivi ottenuti per le infezioni batteriche non lo sono stati per le infezioni fungine per le quali si è in cerca di nuove strategie con l’impiego di nuovi antifungini ma soprattutto con la rivalutazione critica degli approcci terapeutici, compreso lo stesso uso empirico della terapia antimicotica.
